An Athena Clinical Care Associate would be responsible for:
Leadership and Management
To take a lead role in providing leadership and direction to the care team whilst on duty.
•To ensure clinical care monitoring is maintained to the highest standard.
•Assist with the recruitment and training of the care team; ensuring training targets are met and the teams are competent in their roles.
•Assist and inspire the care team to deliver outcomes in care.
•Ensure that all supervision, appraisal and performance management of the care team, supporting them to achieve their potential.
•Act at all times in a professional manner to visitors, staff and clients within the home.
•To attend any Training sessions to further knowledge skills, which are deemed necessary.
•To carry out enhanced clinical care duties; clinical observations of blood pressure, pule, respiration rate, temperature and blood oxygenation level and monitor urine output.
•To assess risk of pressure ulcer development and provide appropriate risk reduction actions, including the application of simple dressings.
•To proactively manage the risk of complications of Diabetes, including the monitoring of blood glucose levels and the administration of prescribed insulin by subcutaneous injection (under the supervision of a competent Registered Nurse).
•To ensure the timely collection of laboratory specimens, including catheter specimens of urine, and blood samples. (Phlebotomy may only be carried out under the supervision of a competent Registered Nurse)
•To carry out the following enhanced administration of medicines practices: the administration of trans-dermal patches, ensuring a witness is present throughout the procedure. Acting as a witness in the setting up of a Syringe Driver for the administration of symptom control medication in palliative care.
•To monitor the risk of malnutrition and dehydration and provide appropriate risk reduction actions including referral to specialist healthcare professionals.
•To assist care delivery in the home, to ensure continuous assessment, planning, implementation and evaluation of resident’s care.
•Assist with ensuring care plans are regularly reviewed and that they are appropriate to the residents needs and reflect the care being delivered.
•Ensure that the person in charge is made aware of any clinical issues or changes in clinical presentation by reporting at 10@10 meetings and recording in the resident care records.
•To ensure the admission and discharge of residents is carried out effectively, taking particular attention to check that items of resident property are accurately recorded.
Training, Skills & Experience Required:
•Must be NVQ level 3 qualified in Health & Social Care or equivalent.
•Minimum of two years experience in a Care position.
•Proven track record for leading, empowering, supporting & motivating a clinical team.
•Full medication training & competencies provided so experience in this is not necessary.
•Training courses available in addition to mandatory training.
