What to expect at your assessment visit
Once you have decided you are interested in moving into one of our care homes we will want to carry out an assessment visit to make sure we can meet your needs and that our Athena Care Home will be right place for you.
We understand that this might feel like a big step, but don’t worry, our team is here to guide you through it. Here’s what you can expect:
Setting up the assessment
- Location – We can conduct the assessment wherever you feel most comfortable, whether that’s in your home, at another care home, in the hospital, or in person at our facility.
- Who can attend – You’re welcome to have anyone you’d like present during the assessment, whether it’s family members or a friend. We want you to feel supported and surrounded by those who matter most to you.
What happens during the assessment
- Getting to know each other – the assessment starts with a friendly chat. It may be carried out by a member of our team who you have met before, perhaps when you visited the home, or by someone new. It’s a chance for us to get to know you better and for you to get to know us better. We’ll ask you some questions, and potentially carry out some simple physical assessments, to understand your needs and preferences better.
- Key details – We’ll need to gather some essential information such as allergies, emergency contacts, GP details, medications, medical history, communication preferences, mobility, nutrition, well-being, social history, religion, lasting power of attorney, and next of kin, so it is useful if you have these things to hand. Some questions might be sensitive, but please know it’s all in the interest of providing you with the best care possible.
- Your questions – this isn’t just a one-way street. Feel free to ask us anything you’d like to know about our care home, the team, the facilities, or anything else on your mind. We’re here to provide you with all the information you need to feel comfortable and confident in your decision. If the team member carrying out the assessment does not have the answers you need they will ensure someone gets back to you as soon as possible.
- Personalised care – We understand that everyone’s needs are unique, so we tailor our care plans accordingly. If there’s anything specific you want to discuss or any concerns you have, please don’t hesitate to let us know. Your comfort and well-being are our top priorities.
After the Assessment
Medical summary – following the assessment, we’ll ask you to contact your GP or your loved one’s GP to request a medical summary. This helps us ensure that we have a comprehensive understanding of your medical history and any ongoing healthcare needs.
Moving into a care home is a significant decision, and we want to make sure you feel fully informed and supported every step of the way.
